USMLE Step 3 

Overview:

  • This is the final step toward obtaining medical licensure in the United States. 
  • It evaluates your ability to apply medical knowledge and manage patients in real-world clinical scenarios. 
  • Step 3 is especially important for international medical graduates (IMGs) and those with an older year of graduation (YOG), as it strengthens their residency application

Exam Format – USMLE Step 3:

  • Duration: 2 days
    • Day 1 – Foundations of Independent Practice (FIP):
      • Approximately 232 multiple-choice questions (MCQs)
      • Focuses on basic medical and scientific principles, including biostatistics, ethics, and patient safety
    • Day 2 – Advanced Clinical Medicine (ACM):
      • Approximately 180 MCQs
      • 13 computer-based case simulations (CCS) assessing clinical decision-making and patient management in real-time scenarios

Exam Timing:

  • Step 3 is available year-round but must be taken at a U.S. testing center.
  • It is recommended to complete Step 2 CK before scheduling Step 3.

Admission Process:

  1. Register through the Federation of State Medical Boards (FSMB).
  2. Provide proof of successful completion of Step 1, Step 2 CK, OET and ECFMG certification.
  3. Once registered, you will receive a scheduling permit, which allows you to book your exam date.
  4. Take the exam at a U.S. testing center (note that there are no centers in Bangladesh).

As of 2025, the USMLE Step 3 exam fees are as follows:

Month 1: Core Review and Clinical Knowledge

Week 1: Foundation of Knowledge

  • UWorld: 30-40 questions/day (Focus on biostatistics, medical ethics, and patient safety).
  • Topics: Internal Medicine, Surgery, Pediatrics, Psychiatry.
  • CCS (Clinical Case Simulations): Practice 2 cases per day.
  • Review: Focus on medical ethics and biostatistics.
  • Start to do the Rapid Review Step 3 courses by Pursuit of USMLE

Week 2: Core Clinical Skills

  • UWorld: 35-40 questions/day (mix of all specialties).
  • CCS: 2 cases/day (Focus on emergency situations and cardiology).
  • Review: Continue practicing clinical decision-making and patient safety.
  • Practice NBME: Take 1 practice test (review weak areas).

Week 3: Strengthening Clinical Knowledge

  • UWorld: 35-40 questions/day.
  • CCS: 3 cases/day (Focus on surgical emergencies and psychiatry).
  • Review: High-yield topics in Internal Medicine and Surgery.
  • Free 120: Take and review questions.

Week 4: Focus on Weak Areas

  • UWorld: 40-45 questions/day.
  • CCS: 3-4 cases/day (Simulate real exam conditions).
  • Review: Revise the weak areas based on your practice exam results.
  • Clinical Decision-Making: Focus on differential diagnosis and management strategies.

    You have successfully completed the Pursuit of USMLE Rapid Review Step 3 course. By now, you should have a strong command of CCS cases, biostatistics, and important foundational topics from Step 1 (First Aid). Your understanding of clinical decision-making and test-taking strategies is much stronger. With consistent revision and focus, you are now well-prepared and confident to take the USMLE Step 3 exam.

Month 2: Final Review and Exam Simulation

Week 5: Consolidate Knowledge

  • UWorld: 40-45 questions/day (Mixed topics, continue focusing on weak areas).
  • CCS: 4 cases/day (Continue practicing with real-world clinical scenarios).
  • Review: Continue reviewing weak areas from UWorld and practice cases.
  • NBME: Take 1 practice test (full-length).

Week 6: Intensive Review

  • UWorld: 40-45 questions/day.
  • CCS: 8 cases/day.
  • Review: Focus on high-yield clinical decision-making and surgical cases.
  • Free 120: Take a second Free 120 to assess progress.

Week 7: Full Exam Simulations

  • UWorld: 35-40 questions/day (Complete last remaining UWorld questions).
  • CCS: Practice 14-15 cases/day (Simulate exam days).
  • Review: Go through clinical guidelines, emergency medicine, and critical care.
  • Full Practice Exam: Take a full-length NBME practice test.

Week 8: Final Preparations

  • UWorld: Light review, focus on last-minute weak spots.
  • CCS: Practice 13-14 cases/day (Refine your timing and management strategies).
  • Free 120: Take the final Free 120.
  • Review: Focus on last-minute high-yield topics and clinical reasoning.
  • Rest: Take a day or two off for mental preparation before the exam.

Key Tips:

  • Biostatistics is your main focus; make sure to review all explanations thoroughly. Revise Step one FA few topics
  • CCS practice is essential: Ensure you’re comfortable with managing real-life cases in a time-constrained environment.
  • Regularly assess your progress with practice exams (NBME/UWSA) and Free 120.
  • Rest: Don’t forget to rest and rejuvenate in the final days before the exam.
